1- COMPOSITION :
ALSAPEN - 1 : each vial contains 1 million U of Penicillin G Sodium .
ALSAPEN - 3 : each vial contains 2 million U of Penicillin G Sodium.
2- PHARMACOPEIA :
ALSAPEN is an antibiotic with a bactericidal action against penicillin susceptible microorganism through the inhibition of biosynthesis of cell-wall mucopeptide.
ALSAPEN belongs to the B-lactams , of the group penicillin G. It is not active against the penicillinase – producing bacteria .
A single intramuscular injection of ALSAPEN injection provides high blood levels but transient .. ALSAPEN bind to serum proteins , mainly albumin , crosses the placental barrier and goes into maternal milk . Biotransformation is weak and it is mainly eliminated in urine. With impaired kidney function , excretion is considerably delayed .
ALSAPEN diffusion in the body fluid is good , but very poor in the tissues .
3- INDICATIONS :
ALSAPEN is indicated in the following :
Streptoccocal , Pneumococcal and Staphylococcal infections . Bacteremia , empyema, severe pneumonia , pericarditis , endocarditis , meningitis , syphilis , anthrax , tetanus, diphteria, erysipeloid endocarditis , rat-bite fever , fusospirochetal infections .
4- CONTRAINDICATIONS :
Allergy to penicillin .
5- PRECAUTIONS :
- Appearance of any manifestation of allergy requires discontinuation of treatment . Severe and sometimes fatal reactions of hypersensitivity ( anaphylaxis ) have been observed in exceptional cases in patients treated with Penicillin .
They are treated with success by epinephrin , steroids , and antihistaminics .
Previous questioning is therefore required before administration .
History of typical allergy to those products constitutes a strict contraindication .
ALSAPEN must be administered in deep intramuscular injections .
- The physician must always investigate and take into consideration eventual allergy to penicillin or cephalosporin before administration of treatment . .
ALSAPEN crosses the placental barrier and goes into maternal milk, so it is not administered to the pregnant or using women.
6- ADVERSE REACTIONS:
Allergic manifestations : fever , urticaria , eosinophilia , quinke’s edema and anaphylactic shock in exceptional cases .
7- REVERSIBLE HEMATOLOGIC REACTIONS :
Anaemia , thrombocytopenia , leukopenia , disorders of coagulation .
8- DRUG INTERACTIONS :
Erythromycin and tetracycline may antagonize the bactericidal effect of penicillin , and concurrent use of these drugs should be avoided .
Concurrent administration of penicillin and probenicid increases and prolongs serum penicillin levels .
Penicillin acts synergistically with gentamycin or tobramycin against many strains of Enterococci .
9- DOSAGE AND ADMINISTRATION :
Severe infections : a minimum of 5 million units daily and up to 20 million units daily is required in divided doses until cure is effected .
