1-COMPOSITION:
MAXIM injection Vials:
MAXIM- 1000 contains:
500 mg ampicilline for injections as the sodium salt B. P.
500 mg cloxacilline for injections as the sodium salt B.P.
MAXIM- 500 contains:
250 mg ampicilline for injections as the sodium salt B.P.
250 mg cloxacilline for injections as the sodium salt B.P.
2-PHARMACOLOGY :
MAXIM is a synergistic bactericidal combination of two semi-synthetic, acid-stable penicillins . The Ampicillin is a broad-spectrum antibiotic, and the cloxacillin is active against penicillin resistant bacteria.
Maxim is effective against the following gram-positive and gram – negative organisms , including penicillinase producing stains .
GRAM- POSITIVE ORGANISMS:
Hemolytic and non-hemolytic streptococci, diplococcus pneumonia, staphylococci (irrespective of their ability to produce beta-lactamase ), clostridia spp, bacillus anthracis. C.xerosis, and enterococci.
GRAM-NEGATIVE ORGANISMS:
H. Influenza, B. Funduliformis, N. Gonorrhoeae, proteus mirabilis, salmonella, shigella, and E. coli.
ABSORPTION / EXCRETION:
MAXIM is stable in the presence of gastric acid, and is quickly and well absorbed when orally administered, producing therapeutically effective serum and urine concentrations.
MAXIM is mainly excreted by the kidneys; around 30% of the oral dose is excreted within 6 hours unchanged in the urine.
When used parenterally, MAXIM produces elevated blood, billiary and urinay concentrations without causing local irritation. MAXIM is non-toxic, usually well tolerated and is only minimally serum bound.
3-INDICATIONS :
MAXIM is indicated in the following therapeutic indications:
1-RESPIRATORY AND E.N.T. TRACT INFECTIONS:
-Pneumonia, bronchitis, lung abscess, empyema, sinusitis, pharangitis, otitis media, tonsillitis, quinsy.
2-GENITO-URINARY TRACT INFECTIONS:
Pyelonephritis, pyelitis, cystitis, urethritis and pelvic infections.
3-SKIN & SOFT TISSUE INFECTIONS:
Cellulitis, abscesses and carbuncles.
4-VARIOUS INFECTIONS:
Septicaemia, endocarditis, pericarditis, osteomyelitis, meningitis,infected wounds, pyoderma, erysipelas, and peri-operative prophylaxis (pre and post-operative infections).
4-CONTRAINDICATIONS:
History of previous hypersensitivity reactions to any of the penicillins or cephalosporins. Pregnant women and nursing mothers should use the drug only under the supervision of a physician.
5-DRUG INTERACTION:
Probenicid may interfere with the excretion of MAXIM
6-PRECAUTIONS :
MAXIM should not be used in the eye either subconjunctivally or locally, and if erythematous rashes develop in patients with suspected glandular fever, treatment must be discontinued.
7-ADVERSE REACTIONS:
As with other penicillins, side effects are usually infrequent, mild and transient. They may include: diarrhoea, indigestion or skin rashes which are indications of hypersensitivity. If an allergy reaction occur MAXIM should be discontinued and the patient treated with the usual agents (pressor amines, corticosteroids, antihistamines, and epinephrine).
8-DOSAGE & ADMINISTRATION:
ADULTS:
Parenterally: 500 mg-1 gr injection every 4-6 hours to be given I.M. In severe cases it is used I.V. by dissolving contents in 10 ml water for injections and administered by slow I.V. (3-4 minutes).
CHILDREN:
Parenterally: 2-10 years of age: 250-500mg lnj. 3 times daily.
1 month-2 years of age: 125-250 mg 3 times daily.or
25-50 mg/kg/body weight/ day divided into 3-4 equal doses.
