1.MECHANISM OF ACTION:
Furosemide is an anthranilic acid derivative which, in common with all diuretics in therapeutic use, primarily brings about the excretion of electrolytes that secondarily, bind water osmotically, and thus increases renal excretion.
It inhibits the active reabsorption of chloride (and sodium) above all in the ascending portion of Henle’s loop.
A consequence of the elevated sodium level in the distal region of the tubule is the increased excretion of Potassium K+.
Furosemide has a very pronounced diuretic action.
Depending on the dosage given, up to 30-40 % of the glomerular filtrate may be excreted, i.e. up to 60 liters within 24 hours.
Contrary to the thiazide diuretics which may not be used when there is a high degree of renal insufficiency.
Furosemide leads to increased diuresis even when glomerular filtration drops below 10 mL/min.
2.INDICATIONS:
Frumix is suitable for the short and long term treatment of disorders in which there is an interstitial accumulation of liquid (Oedema).
In cardiac oedema due to decompensated cardiac insufficiency, congestion of the lungs or liver and oedema of the lower extremities.
In renal oedema, hepatogenic oedema and complications of pregnancy… etc.
Intravenous administration is generally confined to emergency situations, e.g. lung oedema, brain oedema, burns with associated oedema, poisoning.
In case of lung oedema Frumix should be given in a dosage of 1 mg/kg body weight.
20 to 40 mg of Frumix may be administered for initial therapy in cases of brain oedema Infusion of similar doses in electrolyte solutions is suitable for subsequent treatment.
3.CONTRAINDICATIONS:
- Hypersensitivity to Furosemide
- Renal impairment accompanied with anuria.
- Hepatic coma.
- Severe potassium deficiency.
- Hypotension.
- Its use during pregnancy is restricted to life-threatening emergencies.
- Furosemide may inhibit (reduce) lactation.
- Furosemide passes into breast milk. Women for whom Furosemide is indicated should consequently not breast feed.
4.PRECAUTIONS:
Furosemide should be used with care in patients with prostatic hyperplasia and patients with impairment of micturition.
To reduce the risk of ototoxicity, Furosemide should not be injected intervenously at a rate exceeding 4 mg per minute.
5.SIDE EFFECTS:
The most common side effect associated with Furosemide therapy is fluid and electorlyte imbalance such as: hyponatraemia and hypokalaemia.
Signs of electrolyte imbalance include: headache, hypotension, thirsts and gastro-intestinal disturbances.
6.DRUG INTERACTIONS:
- The use of Furosemide in conjunction with aminoglycoside antibiotics (ototoxicity) and Lithium (for a possible increase in plasma – Lithium concentrations) should be avoided.
- It should be given with caution in conjunction with oral anticoagulants.
- Diuretics may enhance the hypotension effect of anti-hypertensive drugs.
- NSAIDs may antagonise the diuretic effect of Furosemide.
7.DOSAGE & ADMINISTRATION:
Frumix is administered orally as tablets or by intramuscular or intravenous injection:
Adults: 20mg – 40mg max 2 to 4 times daily, or 80 mg 1 to 2 times daily.
Children: 1 mg/kg to max 20 mg daily, or according to the physician instructions.
8.PACKAGING & COMPOSITION:
- Frumix 20 – Tablets: A Pack of 20 tablets. Each tablet contains Furosemide 20 mg.
- Frumix 40 – Tablets: A Pack of 20 tablets. Each tablet contains Furosemide 40 mg.
- Frumix 80 – Tablets: A Pack of 20 tablets. Each tablet contains Furosemide 80 mg.
- Frumix 20 – Injection: A Pack of 5 or 25 or 100 ampoules of 2mL. Each 2 mL ampoule contains Furosemide 20 mg.
- Frumix 40 – Injection: A Pack of 5 or 25 or 100 ampoules of 4mL. Each 4 mL ampoule contains Furosemide 40 mg.
9.STORAGE CONDITIONS:
Store Frumix – Tablets and Injection at temperature between (15-30)°C. Protect from light.
