1-CLINICAL PHARMACOLOGY:
MULTIRAXIN activity is due to the suppression of activity in certain key regions of the subcortical area of the central nervous system. Primary skeletal muscle relaxation has been demonstrated with MULTIRAXIN. Bronchodilator activity, and antihistaminic and analgesic effects have been confirmed clinically. An antiemetic effect has been also demonstrated.
Hydroxyzine in therapeutic dosage does not increase gastric secretion or acidity and in most cases has mild antisecretory activity. Hydroxyzine is rapidly absorbed from the gastrointestinal tract and clinical effects are usually noted within 15 to 30 minutes after oral administration.
2-INDICATIONS:
- For symptomatic relief of anxiety and tension associated with psychoneurosis and as an adjunct in organic disease states in which anxiety is manifested.
- Useful in the management of pruritis due to allergic conditions such as chronic urticaria and atopic and contact dermatoses, and in histamine-mediated pruritis.
- As a sedative when used as premedication and following general anesthesia.
3-CONTRAINDICATIONS:
Hydroxyzine is contraindicated for patients who have a previous hypersensitivity to it.
4-PRECAUTIONS:
- Since Hydroxyzine potentiate the action of central nervous system depressants such as narcotics, non-narcotic analgesics and barbiturates, therefore their dosage should be reduced when administered concomitantly with hydroxyzine.
- Patients should be cautioned that the effect of alcohol might be increased when taken with hydroxyzine.
- Since drowsiness may occur with the use of this drug, patients should be warned not to drive a car or operate dangerous machinery while taking MULTIRAXIN.
- Geriatric use: Dose selection for elderly patients should be cautious, usually starting at the low end of the dosing range, reflecting the greater frequency of decreased hepatic, renal or cardiac function, and of concomitant disease or other drug therapy.
- Sedating drugs may cause confusion and over sedation in the elderly patients, so it is advisable to start them on low doses and observe them carefully.
-Pregnancy and lactation: Hydroxyzine should not be given to women in early pregnancy due to suspected fetal abnormalities.
Hydroxyzine should not be given to nursing mothers.
5-DRUG INTERACTIONS:
- Hydroxyzine may potentiate the action of central nervous system depressants such as meperidine and barbiturates, so their use in pre-anesthetic adjunctive therapy should be modified on an individual basis.
- Atropine and other belladonna alkaloids are not affected by the drug.
- Hydroxyzine does not interfere in any way with the action of digitalis and it may be used concurrently with this agent.
6-ADVERSE REACTIONS:
Side effects reported with the administration of MULTIRAXIN are usually mild and transitory in nature.
Anticholinergic: Dry mouth.
Central Nervous System: Drowsiness is usually transitory and may disappear in a few days of continued therapy or upon reduction of the dose. Involuntary motor activity including rare instances of tremor and convulsions have been reported, usually with doses considerably higher than those recommended.
Clinically significant respiratory depression has not been reported at recommended doses.
7-DOSAGE & ADMINISTRATION:
For symptomatic relief of anxiety and tension associated with psychoneurosis and as an adjunct in organic disease states in which anxiety is manifested; in adults, 50-100mg 4 times daily; children under 6 years, 50 mg daily in divided doses and over 6 years, 50-100 mg daily in divided doses.
For use in the management of pruritis due to allergic conditions such as chronic urticaria and atopic and contact dermatoses, and in histamine-mediated pruritis; in adults 25 mg 3 or 4 times daily; children under 6 years, 50 mg daily in divided doses and over 6 years, 50-100 mg daily in divided doses.
As a sedative when used as a premadication and following general anesthesia; 50-100 mg in adults, and 0.6 mg/kg in children.
8-OVERDOSAGE:
The most common manifestation of MULTIRAXIN overdosage is hypersedation.
If vomiting has not occurred spontaneously, it should be induced. Immediate gastric lavage is also recommended. General supportive care, including frequent monitoring of vital signs and close observation of the patient is indicated.
Hypotension, though unlikely, may be controlled with intravenous fluids and levarterenol, or metaraminol. Do not use epinephrine as MULTIRAXIN counteracts its pressor activity.
If barbiturates have been ingested concomitantly, hemodialysis may be indicated.
9-PACKAGING & COMPOSITION:
MULTIRAXIN – Syrup: A pack of glass bottle of 100 mL. Each 5 mL contains Hydroxyzine HCl 10 mg.
10-STORAGE CONDITIONS:
Store MULTIRAXIN – Syrup at temperature between (15-30)°C. Protect from light.
