1-PHARAMCOLOGY:
The pain of migraine is due to pulsating of the extra cranial arteries which have become dileted. Constriction of these vessels by drug usually relieves the pain. And the spasm of the striated muscles is often a cause of pain including some headache.
Paracetamol has a peripherally acting analgesic.
Ergotamine tartrate is an alpha adrenergic blocking agent with a direct stimulating effect on the smooth muscles of peripheral and cranial blood vessels and produces depression of central vasomotor centers .
The compound also has the properties of serotonin antagonism.
Caffeine also a cranial vasoconstrictor , enhance vasoconstrictive effect without the necessity of increasing Ergotamine tartrate dosage.
Meprobamate has been shown to have effects at multiple sites in the central nervous system , including centrally acting muscle relaxant .
Phenobarbital reduces a high degree of sedation.
Belladonna specific inhibitors of the parasympathetic system , reinforced by the synergistic action of Phenobarbital in dampening the cortical centers.
2-INDICATONS:
Migraine headache attacks (migraine) .
3-CONTRAINDICATIONS:
Hyper sensitivity to the product.
The product should be avoided in pregnancy and used cautiously in peripheral vascular and coronary diseases and hepatic insufficiency .
Hyper tension or renal disease .
4-DOSAGE :
One or two tablets at the onset of the prodromal symptoms .
5-ADVERS REACTIONS:
The drug may cause nausea , vomiting , somnolence , and peripheral vascular spasms .
6-DRUG INTERACTIONS :
Increase of Ergotamine construction in plasma have been reported in patients given Erythromycin in addition to Ergotamine.
Peripheral vasoconstriction was reported in patients when coadministered with Propranolol. The product should not be coadministered Sumatriptan or Zolmitriptan .
7-WARNINGS:
Prolonged use of this drug may in rare cases cause allergic reactions and decrease in the number of leucocytes as all pyrazolone derivatives.
Therefor leucocytes count must be done and drug administration this continued if patients should be warned not to drive cars or operate machinery .
