1-PHARMACOLOGY:
PULMO-NEB (Salbutamol) is a direct acting sympathomimetic agent with predominantly β-adrenergic activity and a selective action on β2 receptors, where as β - receptors activation increases the influx of calcium across the cell membrane and its sequestration inside the cell. β – receptor activation also promotes the relaxation of smooth muscles.
PULMO-NEB (Salbutamol) has exhibited to possess more prolonged and selective actions than the other similar drugs as a predominantly β2 receptor stimulant, its bronchodilating action being related more prominent than its effect on the cardiovascular system.
2-INDICATIONS:
PULMO-NEB is used in the treatment and prophylaxis of:
-All forms Bronchospasm in asthma.
-Chronic bronchitis and bronchiolitis.
3- SIDE EFFECTS:
Tell your doctor if you notice any of the following:
Difficulty breathing or worsening of your breathing problems, swelling or severe rash, fast or irregular heartbeat, pounding heart beat.
Few Incidences of side effects have been reported following administration of Salbutamol including:
Allergic reactions, headache, dry mouth, drossiness, gastroenteritis, rash or itching skin, cold symptoms, nausea or vomiting, tremors, headache, insomnia, hypertension, bronchospasm, wheeze, cough, dyspnea, dyspepsia, systemic malaise.
4-WARNINGS:
Tell your doctor:
- If you have any allergies to any other medicines or to any other substances, such as foods, preservatives or dyes.
- If you are pregnant or intend to become pregnant.
- If you are breast feeding or plan to breast feed.
- If you have any medical condition, especially the following: Thyroid problems, heart problems, high blood pressure, diabetes, glaucoma.
As with other inhaled β-adrenergic agonists, Salbutamol sulfate inhalation solution can produce paradoxical bronchospasm, which can be life threatening. If it occurs, the preparation should be discontinued immediately and alternative therapy instituted. Fatalities have been reported in association with excessive use of inhaled sympathomimetic drugs and with the home use of nebulizers. It is, therefore, essential that the physician instruct the patient in the need for further evaluation, if his/her asthma becomes worse. In individual patients, any β2-adrenergic agonist, including salbutamol solution for inhalation, may have a clinically significant cardiac effect.
Immediate hypersensitivity reactions may occur after administration of salbutamol as demonstrated by rare cases of urticaria, angioedema, rash, bronchospasm, and oropharyngeal edema.
5-DRUG INTERACTIONS:
-Salbutamol should not be given concomitantly with other beta-receptors stimulants such as epinephrine.
-Asthma patients should not be treated with beta blockers like propranolol, and if there is no alternatives in certain cases (e.g. prophylaxis after myocardial infraction) a cardio-selective beta blockers may be given with caution.
-PULMO-NEB should be given with caution in patients receiving monoamino oxidase inhibitors or tricyclic antidepressants or in 2 weeks of discontinuation of such agents.
6-PREGNANCY:
Pregnancy category: C.
PULMO-NEB should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus.
7-NURSING MOTHERS:
A decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the importance of the drug to the mother.
8-DOSAGE & ADMINISTRATION:
The dosage depends on patient condition and the concomitant therapies.
Children over 4 years: Usual dose is 2.5 mg given by nebulizer repeated every 4-6 hours.
Adults: Usual dose is 5 mg given by nebulizer repeated every 4-6 hours.
NOTICE: Fresh solution must be used for each dose. After the full dose has been given, any solution remaining in the nebulisers must be thrown away. Nebulisers must be cleaned after use.
9-OVERDOSAGE:
Manifestations of overdosage may include seizures, anginal pain, hypertension, hypokalemia, tachycardia, with rates up to 200 beats/min. In isolated cases in children 2 to 12 years of age, tachycardia with rates >200 beats/min has been observed.
10-PACKAGING AND COMPOSITION:
PULMO-NEB – Inhalation Solution: A pack of 7 or 14 plastic unit dose ampoules of 2.5mL. Each plastic ampoule contains 2.5 mg of Salbutamol (Sulfate).
11-STORAGE:
Store below 25°C and protect from light.
